This Event Toolkit includes the resources you need for a successful event. Be sure to review all of the documents described below. The toolkit includes:

• Meeting Planner's Checklist: From securing the speaker and facilitating the event, to follow-up activities, use this checklist as your planning roadmap. Click here to download the checklist.
  
  
• Activity Request/Promotion Form: Have an idea for an event? Complete this form and submit to wellnessworks@howardcountymd.gov. Click here to download the form.
  
  
• Instructor Credentials/Program Risk Guidelines: This file includes recommended certifications for exercise instructors, as well as "level of risk" guidelines for evaluating whether to offer a specific program and the applicable waiver forms and required approvals. • Waiver Form: Employees must complete this form before participating in any off-duty personal fitness or recreational activities. Click here to download the form.
  
  
• Evaluation Form: Be sure to bring copies of this form to your event and ask participants to complete. It's important to track our results and continue to improve our programs. Click here to download the form.
